Housecalls:
If you are unable travel to the clinic, I am happy to make housecalls. I charge a flat rate for housecalls that covers my time and travel expenses to your home.

Conveniently located in central Seattle off Eastlake Avenue, a short distance from I-5 and downtown.
Parking:
There is usually ample street parking on Eastlake Avenue until 4 PM, when parking enforcement begins towing to accommodate rush hour traffic. Parking on Eastlake Avenue may resume at 6 PM. There is also parking along side streets and five stalls in the office garage (parking slots are labelled 'ACW' and are first-come, first-serve).

Download forms (health history questionnaire, consent to treatment, and privacy policy). You can print these forms out and complete them at your convenience prior to your first visit.
